Completing the insurance application form:

Start date for the insurance
Duration of the insurance (in months)
Gender
If you tick female the system opens a question:
"Are you pregnant or do you plan on getting pregnant during the term of the insurance?"
Are you a student?
At this point the system will generate a price for the insurance.

If you wish to make the insurance contract the you need to complete the data in the rest of the form.

Social security number or other number of the insured (Answer: Neni or none)

Address of place of reported stay (Czech address used for your visa application/residence permit)
Email address (This is important, you need to use the eamil address where you want the insurance documents sent to)
Phone number (you do not need to complete)
Tick the box, bottom right: "Payment" and the system take you to the payment page where you can pay by credit card.
Once you have paid the system will automatically email you the docuemnts you need for your visa collection

IF you are unsure about completing the insurance company form, we can fill in the form and send you all the insurance documents We can complete the for for you. And send you the insurance documents to any email addrtess you want.
There is no extra charge but we will need:
at least 24 hrs before you need the insurance
payment for the insurance
a copy of the information page of your passport
the Czech address you used for your vis/residence permit application
a start and end date for the insurance

For the embbassy you will need:

A copy of the contract
The proof of payment
A copy of the health insurance card

Axa have a very specific health insurance plans that meet Czech visa reglations.

These plans differ from International Axa health insurance plans.
The Czech health insurance plans are split into to categories. Basic and Complex helath insurance.
Axa Basic health insurance is suitable for collection of a Czech visa if you will be employed in CR on a Czech employment contract.
Basic health insurance cover emergency treatment only. The insurance does not cover standard visist to the doctor.

Axa have a short list of contracted doctors or clinics, you should not have to pay anything at a contracted clinic.
With Axa you will most likely pay cash for your treatment. Tell the doctor or clinic you will be paying cash, this should not be a problem.
Ask for a receipt and claim the money back by contacting the assistance sevice.

When you call, there will be some answering service/machine.
Press 1 for Czech....
Press 2 for...
Press 3 for...
Press 4 for communication in English.

When someone answers they will most probably speak to you in Czech first. Just speak English to them and they will speak English. Tell them you have Axa Complex health insurance and they will need your contract number. So have your contrct number ready.
